Easing SIJ Pain: How Deep Tissue Massage Supports Lower Back Health - Techniques & Modalities Series

Andrea Hayes ·

Okay, this one technically isn't a Techniques & Modalities blog but it is one that I see so often and crazily enough I suffer from this myself.

I went for a run the other day, yes, a run.. well a run walk, taking it slowly to build up fitness again.  And wow, one day later, I was in a world of pain.  I have done all my self massage, massage ball, stretching and mobility exercises but nothing is helping.  I ended up taking an ibuprofen last night to help me sleep.  It isn't perfect today, but it is better.

In this blog, I'll explain what SIJ dysfunction is, how deep tissue massage can help, and why a tailored, hands-on approach is key to lasting relief.

If you suffer from ongoing lower back pain, you’ve likely heard terms like SIJ dysfunction or sacroiliac joint pain without really knowing what they mean, or more importantly, what can help. At BSPOKR Remedial Massage, we see this issue often, and one of the most effective treatments is targeted deep tissue massage.

For me this is lower than lower back pain it is most definitely, in upper glutes and refers down the sides into legs.  Painful to touch when inflamed.

What Is SIJ Dysfunction?

Your sacroiliac joints (SIJ) sit where your spine meets your pelvis. They play a crucial role in transferring weight between your upper body and legs. When these joints become dysfunctional, from injury, instability, pregnancy, or poor movement patterns, pain can radiate through your lower back, hips, and even down into your legs.

Signs of SIJ Pain:

  • Localised pain near the dimples of your lower back

  • Pain when standing up from sitting

  • Discomfort climbing stairs, walking uphill, or rolling in bed

  • Stiffness or ache through hips and glutes

SIJ dysfunction is often confused with sciatica, disc issues, or hip problems, but skilled assessment (which we include in your remedial treatment) can help pinpoint the true cause.

How Deep Tissue Massage Helps SIJ Dysfunction

When your SIJ is irritated, it’s rarely just about the joint itself, surrounding muscles often become tight, overworked, or imbalanced as they try to stabilise the area. Over time, this protective tension can worsen pain and restrict movement.

Deep tissue massage works to relieve these muscular imbalances. It focuses on breaking up adhesions, releasing chronic tension, and restoring normal muscle length. By softening and rebalancing key muscle groups, we reduce the strain on your SIJ and allow the body to move more freely.

Muscles Commonly Treated for SIJ Pain

At BSPOKR, your SIJ-focused treatment may include working into:

  • Gluteus medius and maximus (hip stabilisers)

  • Piriformis (often implicated in referred pain patterns)

  • Quadratus lumborum (QL) (lower back support)

  • Hamstrings (linked to pelvic position and mobility)

  • Hip flexors (which influence pelvic tilt)

Techniques We May Use

Beyond deep tissue, we might blend in:

  • Trigger point therapy for releasing stubborn muscle knots

  • Myofascial release to free up connective tissue restrictions

  • Stretching and mobility work to restore range of motion

These techniques are adjusted based on whether you’re in an acute flare-up or working on long-term correction.
